We want your enrollment to be as smooth as possible. Here is a step-by-step guide on how to join Marin Family Medicine.

1. Make Sure We’ve Met

We want to ensure our practice is the right fit for your needs.

• If we have already spoken: Great! Please proceed to Step 2.

• If we haven't connected yet: Let’s talk first. Sign up for a Meet & Greet Call so that we can explain how Direct Primary Care works and answer any questions.

2. Create Your Membership

Once you click the button below, you will be asked to provide basic information about yourself, provide payment information, and sign our Patient Agreement. Please read the agreement carefully. We will also review the most important points at your first visit.

A note about billing:

• Initiation fee: $1,000 per adult (no initiation fee for kids), capped at two per family. This is billed on the day of your first visit.

• Monthly membership: Your first monthly fee will be billed one month after your first visit.

For families, our enrollment system sometimes overestimates your initiation fees—we'll review and correct this before any charges go through.

3. Schedule your initial visit

After you’ve enrolled, you will be prompted to schedule your initial visit with us so we can get to know your health history and goals.
